AI agents can produce correct-sounding answers while leaving the underlying workflow in an invalid state.
For a CTO or Head of AI, that is the gap between an agent that performs well in a demo and one the business can safely place inside a real operating process. The answer may look right while the next system receives no enforceable decision, recovery path, or authority boundary.
An agent may recognize that an action is unsafe, yet fail to communicate that decision in a form another system can inspect and enforce. It may omit the evidence behind the decision, ignore missing approvals, skip required tools, or cross an authority boundary.
We tested whether those failures could become a reviewed, installable improvement without retraining or replacing the model.
THE HORIZONTAL PROTOCOL LAYER
Your stack can run, observe, and score an agent. Dharma makes the consequential handoff inspectable and governable.
These systems answer different questions. Dharma connects them around the transition that must carry evidence, delegated authority, restrictions, and recovery state from one system to the next.
Architecture boundary: the runtime gate validates structure and configured authority before a bounded handoff. Semantic judging happens on the recorded trajectory and informs evaluation and remediation; it is not represented here as a real-time release authority. Adapter support and equivalent enforcement remain deployment-specific.
Agent Fabric packages this lifecycle across connected local and managed agents: signed task authority, bounded state handoffs, trajectory capture, deterministic checks, semantic evaluation, Failure Atlas clustering, GitHub-reviewed remediation, held-out gates, signed installation receipts, and rollback.
THE CONSEQUENTIAL DECISION
The answer looked obvious. The workflow was not.
The evaluated agent received a release request containing contradictory evidence. Support asserted that an image-orientation fix was ready. The supplied CI and approval evidence established that it was not.

WHY THIS FAILED
The model reached a reasonable conclusion. The business did not receive a valid transition.
- Generic
respondinstead ofblock_release. - No state envelope or machine-readable evidence boundary.
- No explicit record of missing approval.
- No permitted, prohibited, or recovery actions.
- Required-tool and false-premise checks failed.
STATE ENVELOPE
Make the action boundary inspectable before execution.
A state envelope records what the agent intends to do, what it knows, what remains unknown, which evidence it used, which authority applies, and which actions are allowed or blocked.
The integrity contract defines the conditions that proposed action must satisfy. JSON alone is not authority. The envelope must pass that contract and be accepted by the receiving control.
The state envelope carries the evidence. Deterministic runtime policy checks its structure and delegated authority. The receiving system can enforce configured boundaries without reinterpreting prose, while semantic evaluation remains a separate post-trajectory control.

The workflow receives a decision it can enforce.
The governed agent did not merely explain the blockers. It issued an explicit action with evidence-bound reasons and recovery requirements.

VALIDATION BEFORE ACTION
The runtime gate rejected the first envelope.
In this preserved July evaluation, the governed agent did not produce a structurally valid state envelope on its first attempt. A deterministic runtime gate rejected it and accepted the corrected envelope before the action submission continued. This proves configured structural enforcement, not a real-time semantic judgment of the rationale.
Submit the state envelope
Required authority context is missing
Add evidence, constraints, and allowed actions
Block the release and record the decision
The runtime did not ask the model to “be more careful.” It enforced the required state contract, while the evaluation layer turned the resulting trajectory into evidence for remediation.
FAILURE → VERSIONED CONTROL
The Cognitive Integrity loop turned twenty-seven failures into an inspectable remediation.
Across the source evaluation, failures contained one or more of five reproducible scorer flags. Dharma converted those flags into explicit behavioral requirements carried by the Cognitive Integrity Protocol.

RELEASE GATE
The integrity contract required proof before installation.
A plausible remediation was not enough. The intervention could not become active until it passed a separate held-out backtest and retained human approval.

HELD-OUT RESULT
30/30 PASS Installation remained approval-gated.WHAT THE RESULT MEANS
Cognitive Integrity improved behavior without expanding authority.
The model did not become more knowledgeable. Under the Cognitive Integrity Protocol, it became more reliable at deciding when its evidence and authority were sufficient to act.
WHAT CHANGED
- Explicit evidence and authority rules
- Required tool use
- Complete state before action
- Preserved blocked actions
- Versioned, testable intervention
WHAT DID NOT
- The underlying model
- The model’s domain knowledge
- The agent’s permitted authority
- The held-out answers
- Human approval before installation
COGNITIVE INTEGRITY IN PRACTICE / THE SUPPORTED CLAIM
Using the same underlying model, the agent passed 3 of 30 source evaluation tasks before remediation and 30 of 30 separate held-out tasks after a reviewed, versioned remediation was applied.
Evidence basis: preserved production-path campaign records, distinct source and held-out task packages, an immutable installed skill revision, and the retained 30-task held-out result. The comparison is bounded to this disclosed synthetic benchmark.
FROM PLAUSIBLE OUTPUT TO DEPENDABLE ACTION
Most evaluations stop at the answer.
Cognitive Integrity asks a more demanding question: did the agent preserve the state, evidence, authority, and action boundaries required for the business to proceed?
In the baseline example, the words were reasonable but the workflow remained incomplete. With Dharma, the runtime could inspect the structured handoff, reject an invalid envelope against configured policy, and record the corrected action. The resulting trajectory could then be semantically evaluated and used to propose a reviewed remediation.
For technical buyers, the value is operational: less ambiguity between systems, explicit release and recovery conditions, and a controlled way to improve agents from observed failures without retraining the model or widening its authority.
Your stack makes agents capable. Dharma makes their consequential actions governable.
